Wesley Tingle has no release form on file. Next time he comes in for his leg piece appointment, have him fill out a release form so we have his address for records. He's a recurring client of Dave's with at least 4 sessions logged (Mar 2025, Jul 2025, Nov 2025, Feb 2026).
